Reactivate USDOT Number

If you need to reactivate your USDOT Number you can do so now online.

After you have submitted this request, you will be sent over paperwork with instructions on how to fill it out. Please fill it out and reply back to the agent with that form so that they can process it. Delays with receiving this paperwork may cause your USDOT Number Reactivation to be delayed. This is only for carriers who have previously voluntarily deactivated their USDOT Number, your USDOT Number has been revoked or your USDOT Number shows as "Inactive". This is not for carriers who have an Out Of Service USDOT Number or are labeled as temporarily inactive due to lapse in insurance or late filing of the MCS-150.

Please note, it can take anywhere from 1 week to 3 weeks once an agent has processed your request for your USDOT Number to be considered active.
